San Pablo City National High School Annex V was established in 1996, under the San Pablo City National High School Main.  With the help of the City Council under the leadership of our City Mayor Hon. Vicente B. Amante, Hon. Lauro Vidal – Vice Mayor, City Counselors Pablo Ticzon, Eduardo U. Dizon, Schools Division Superintendent, OIC, Dr. Miralinda B. Ocampo, and Principal of San Pablo City National High School Main, Mr. Ricardo T. Manalo began developing SPCNHS Annex V.

The school’s land is the family gift of the late Don Tomas D. Dizon, of his former mayor, Cesar Dizon and its heirs. It started with about 2000 students and 37 teachers.  Despite the lack of classrooms, all subjects were taught.  Due to the increasing number of students, Mr. Ricardo T. Manalo called for Teachers-in-Charge (TIC) from Mrs. Julieta Donato, Mrs. Marilou Bicomong, Mrs. Elsa L. Ilagan, Mr. Ely Flores and Mrs. Neliza P. Cuasay.  The resolution was passed by the City Council, and SPCNHS Annex V became Col. Lauro D. Dizon Memorial National High School (CLDDMNHS) in 2005 under Mrs. Evelyn C. Malabag (2004-2011). Mrs. Cristeta S. Uy assumed the responsibility of the school from 2011 – 2016 and was followed by Mr.  Albert T. Saul from 2016- 2017.  Mr. Patrick L. Ilagan became the next school head at the same time creating a name and brand in society in its new name, Col. Lauro D. Dizon Memorial Integrated High School (CLDDMIHS) 2015-2021. At present, the school was headed by the dynamic Mrs. Editha L. Fule.
